• Once the form is submitted a brief questionnaire will automatically be sent to the email address you provided. 

• Please utilize this questionnaire to provide additional details regarding your custom order. 

• Once submitted, your questionnaire will be reviewed and we will contact you if we have any additional questions.  

• If there are no additional questions your order will be confirmed and an invoice will be sent to the email address you provided. 

• All invoices must be paid in full before production begins. 

• If your invoice is not paid in full within 5 days of receipt, your order will be cancelled.
